Encashing UK Pension

I have a small pension (greater than 30K) from the UK that I'm cashing in on the basis of ill health, I need to have a Pension Advice Document signed by a Financial Adviser who is registered with the UK FSA. I've tried unbiased.co.uk without success.

Does anybody know of a FSA approved financial adviser in Ireland (or the UK) who can sign the PAD?
