thanks

This form has indeed changed. We use ICC Company formation (no affiliation). €199 all in fee and you get a company register and company seal. They must have an internal peace commissioner or something because its never an issue with them.

what is the form now called??

The form hasnt changed. It is still called the A1 and [broken link removed]

Note 14 explains that the form must
be signed by either a solicitor who is forming the company or the
director or secretary in the presence of either a commissioner of oaths, notary public, solicitor or peace
commissioner.

So... You can bring this form to a solicitor who will sign as declarant, or if you bring it to a commissioner of oats, peace commissioner or notary public, you sign it in their pressence.

From memory, you hadn't to get it signed by a solicitor or a peace commissioner.
