Claiming Rent Relief for Private Rented Accommodation

I'm leaving a property today and want to claim this relief.
I paid monthly rent to the person who held the lease, not to him directly. The owner was fairly slow to get snagging problems sorted out and I'm not hopeful that he'll fill in the necessary details on this form. Is he obliged to do so and is their a deadline by which he must return the form to me?
 

If he won't fill out the form the n send the form in iwth as much detail as you can. Revenue will grant you the relief as long as you have landlord's name and address on the Rent 1 form. Having the PPS number of the landlord just make's it easier for revenue to identify the landlord but it's not essential
