Claiming PRSI relief form CGPRSI1?

I'm filling out Form CGPRSI1 to claim back the PRSI relief on my RAC contributions which I pay monthly.

The form asks for a date of payment but it only has room for three payments so do I print four forms off the revenue website thereby giving me 12 payment dates, one for each month, or do I enter the total contributions for the year on one form and leave the date blank?

I did mine yesterday and posted it off. I actually wrote in the dates 1/1/2008 - 31/12/2008. We'll see if it comes back to me.

I called revenue about this and they said to fill in the total for the years contributions and just write 'monthly' by the the date.

I reckon you'll be OK!
