Does a child who's surname is both parents surnames (Double Barrel), need both parents permission to get a passport if they were never married.

Yes. Regardless of the child's name, both parents must sign the form.
 
If the parents were never married and the mother is the sole guardian then the mother can sign an oath swearing that she is the sole guardian, witnessed by a solicitor. The passport form is to be signed and witnessed by solicitor and the childs birth cert also.
