I used my BOI MasterCard 3 times on holiday to withdraw cash. On 2 occasions I withdrew €300, and the charge was €4.50 each time. I also withdrew €90, but the fee for this was €2.54.

Why is this?

You used it to do cash withdrawals (from a preloaded account) or a cash advances? How were the charges described on your bill? Were these € zone withdrawals? What do the terms & conditions of your card agreement say about charges?
 
Ah - just checked the latest IFSRA CC charges survey and it seems that BOI MC cash advance charges are 1.5% subject to a minumum of €2.54 which probably explains it?

All withdrawals were cash advances

On the statement they are described as "Cash Advance Fees"

Yes, they were eurozone withdrawals (Gran Canaria)

In the past, I have only been charged €2.54 for cash withdrawals in other eurozone countries. I have not yet queried this with BOI.
