Just a query

Had serious enough surgery in 2005 , had a botched operation and then had another emergency op , and then had septicimia set back due to the complications etc. i was told that i would have further surgery to reverse the symptoms of the disease.
As u can guess life altering diseases put a different view point on your life
Needless to say but i feel i need to try something i always wanted to do
And now i have a chance of applying for a job.

All these jobs say when it comes to the medical stage "these may include a medical/health check to ensure that the prospective employee is capable of doing the job in the foreseeable future "

But this is the thing i am fit to do the job i 'm doing now
I came back to wrk after been out sick for a long time to become employee of the year. no one could say that i have been thru what i have had .
Yet if i go for this job and i end up getting it

And the consultant says , i think u are ready for the nxt bout of surgery

Would i be better off to tell them at the interview First stage of my current position
Or should i be quiet on the First hurdle , and if i am lucky to get second interview mention it then

Or do i wait until the medical , once the doc sees me he 'll know when i give him my history

Please advise ?

I would wait until the medical - no point in making it harder for yourself. I've heard some interesting questions about the validity of pre-employment medicals, except where they are checking for specific abilities relevant to the job in question. To reject somebody from a finding in a general medical could put an employer on very dodgy ground.
