Apply for a new passport after old one lost?

I lost my passport on the bus and the Lost and Found office didn't have it when I called them. I need a new one but apparently, I have to get it reported to the Gardai. Do I write a statement and then get a guard to sign it off or do they have a specific form that I'll fill in?
