Affidavit of means - whos sees it OR sould know its contents?

Can anyone tell me who gets to see the affidavit of means prepared by a spouse who is looking for a separation?

I have a fear of completing this in case is is shown/ the contents disclosed or a copy could be given to a 3rd party i.e. a Bank.

A copy of the document is sent to the other party. You will have no control over what they do with it.

Bear in mind that this is a legal document which you swear to be true to the best of your knowledge and belief.

If you're telling everyone the truth there shouldn't be any concern.
