Advise on laying a footpath

Hi,
Would anyone be able to advise me on laying a foot path (around a house).

Just a couple of questions
- How wide should it be
- How much of a fall from the house should it have?
- Also would I use pegs and boards to hold the cement in place to try?
- Would I lay gravel and then add the cement?

The questions are probably stupid as the quote i got to get them done by a builder, so im giving it a bash

Any help would be great
