Additional Income - form 11 or 12?

Hi,

I'm beginning to think about my 2008 tax return now (it'll be my first).

I am a PAYE worker with very modest additional income - circa €200 dividend income in 2008 (post WHT).

I also bought and sold somes shares in 2008, but didn't exceed the €1,270 CGT profit threshold.

From a perusal of the Revenue website, I am confused as to whether it is Form 11 or From 12 I should be filling in to declare this additional income (and declare the CGT disposal). From what I can see, these forms cover the same ground. What is the difference between them?

If anyone could clarify I'd be much obliged.

The form 11 is for people with material non-PAYE income. The form 12 is for PAYE earners with some additional income. From the facts as you described, a Form 12 would be more appropriate to your circumstances.
 
As you are an employee and most of your income is derived from that employment you should use form 12.
Form 11 is used mainly by self-employed individuals.
