Chasing Unpaid Rent and Bills

I submitted a civil liability summons for this, and the defendant did not issue a form stating his intent to defend. The district court clerk said I must now submit an affidavit and a decree. I looked on 'courts.ie' but there are alot of types of affadavits and decrees on that website. Could anyone please tell me the numbers of the affidavits and decrees that would be applicable in this situation.

Why don't you go down to the local district court office and ask the staff there, they are very helpful.

I did, and believe it or not, they don't know. I'll probably just download from the internet the two forms that I think are the most accurate and hope they are okay.
