Changing folio to joint names

My mother inherited her home from a relative. The folio was transferred into her name. As she and my father are getting on in years a bit they want to ensure everything is neat and tidy and want to move the folio into their joint names.

I have spent a while looking at the various forms on the PRAI website but am struggling to figure out which one is appropriate.

Any insight much appreciated!
